The National Transportation Safety Board has released its preliminary report for the May 3 accident in which a low-flying United Airlines Boeing 767-400ER struck a light pole on the New Jersey Turnpike while on final approach to runway 29 at Newark Liberty International Airport.
Debris from the light pole subsequently impacted a tractor-trailer, causing minor injuries to the driver. There were no injuries among the 220 passengers or 11 crewmembers aboard the United flight, which was inbound from Venice, Italy.
The preliminary report discusses the captain’s preference for flying the approach to runway 29, which does not have an instrument landing system, slightly below the guidance provided by the runway’s precision approach path indicator (PAPI), a type of visual guidance lighting system. According to the NTSB report, United has since warned all of its pilots that “the pilot technique of shifting the aimpoint of electronic or visual glide path indications, or ‘ducking under’, contributes to low approach altitudes during the visual segment into certain airports and runways.”
