The NTSB today issued 50 total safety recommendations to the FAA, Department of Defense and other entities following a day-long recounting of the January 2025 fatal midair collision between a U.S. Army Black Hawk helicopter and an American Eagle CRJ700 regional jet which killed 67 people.
The Board by a unanimous vote determined the midair collision was caused by a series of “systemic failures” that placed a helicopter route too close to the final approach path for a runway at Washington Reagan National Airport, largely confirming reporting from The Air Current in December.
The Board is recommending the FAA tackle issues ranging from how it classifies and pays air traffic controllers and determines airport traffic rates to conducting a more thorough review of helicopter routes near Washington, D.C. Recommendations were made to the U.S. Army concerning its safety reporting systems, pilot training procedures and use of ADS-B location tracking technology.
The Board’s final report, which Chair Jennifer Homendy said totals over 500 pages, will be released within two weeks.
